FOSTER CHILD VISA APPLICANTS WHO ARE UNDER 18 ENTRUSTED TO 

RESIDENTS IN QINGDAO BY CHINESE WITH FOREIGN 

NATIONALITIES/OVERSEAS CHINESE MUST PROVIDE:

1.	VISA APPLICATION FORM or RESIDENCE PERMIT 

APPLICATION FORM which is fully completed , and one recently 

Taken, two-inch,color passport photo without a hat;

2.	TEMPORARY RESIDENCE REGISTRATION FORM;

3.	Valid passport,both the original and a photocopy,a 

Valid visa , both the original and a photocopy,last time 

Entry information page,both the original and photocopy;

4.	Birth certificate of the foster child,copies of the 

Parents’ passports , proof of foreign country residence if 

Father and/or mother are Chinese;

5.	Parents’ Letter of Entrustment designating 

Guardians,term of entrustment,etc.;

6.	Guardian’s ID,notarised letter of guarantee which 

Must acknowledge acceptance of parents’ entrustment on the 

Part of the guardian who undertakes fosterage and 

Guardianship and guarantees that the children under 

Fosterage will leave China as it is arranged in the terms of 

The entrustment;

7.	Proofs by oversea organizations must be verified by 

The Chinese Embassy or Consulates and with Chinese 

Translation stamped by the translation company .
